A Plan of Care is a road map to meet the client’s needs. It includes recommendations for resources and services to assist in managing a client’s unique situation. The Plan of Care is based on the comprehensive assessment with the focus on helping the client to live at their highest functional level in the least restrictive environment possible.
An Individualized Plan of Care can include the following recommendations:
- Appropriate living environment
- Medical providers
- Durable medical equipment (DME)
- Transportation services
- Medication management
- Companion/Caregiver services
- Home Health services
- Financial and legal professionals
- Family support services
The Plan of Care can then be implemented by the Life Care Professional, the family or a combination of both. Each situation is different and we will work with you to determine the best method of implementation for your situation.
